BIRMINGHAM ANTICOAGULATION PROGRAM
FOR PRIMARY CARE

BAP-PC is a Clinical Decision Support System (CDSS), i.e. software which translates INR (obtained via a blood sample by means of a laboratory or near patient testing device such as Coaguchek or Thrombotrak etc) into a warfarin dose. The system is being used in practices in the UK and Ireland. The software has been developed over a 8-year period and is aimed primarily at facilitating anticoagulant dosing in Primary Care. At the heart of the software is the Coventry Equation, a published mathematical algorithm which has been validated in both Primary Care and General Practice.

In Europe and the UK increasing numbers of patients on warfarin are going in for self-management of oral anticoagulation. BAP-PC is being used in primary care and has the potential to also be used for patient self-management.

BAP-PC 3, our latest upgrade, is an extensive rewrite incorporating many suggestions from our users in the UK and Ireland. The system enables users to set up and run clinics, it suggests a dosage and review periods.

Quality Assurance

BAP-PC allows users to perform comprehensive audits on both individual patients and the patient population. The software is a capable of identifying poorly performing patients and can generate a variety of reports from the data.

It also provides facilities to record Internal Quality Controls (IQC) and External Quality Assurances (EQA) both of which are aimed at ensuring good practice.

Users subscribing to technical support are also invited to participate in a unique External Quality Assessment scheme for warfarin dosing run by the department for users of BAP-PC, where user data are audited and analysed by us and compared to the results of other users of the software.

BAP-PC: This software provides advice on Warfarin dose and recall date calculation and relies on the user applying clinical judgement. It is not a substitute for that clinical judgement.
